Access to Vital Records:

A requestor of a certified vital record must provide positive identification including, but not limited to, a driver’s license, passport or other government issued picture identification.  Requestors who are immediate family members must provide proof of lineage, if not listed on the record being requested.  Requestors who are not immediate family members must also provide written proof that they have a direct and legitimate interest in a record before one can be issued to them.  Someone who is not entitled to a record as a family member or who cannot prove a direct and legitimate interest can complete and submit a Release of Vital Records Application.

Requests by Mail:

Certified copies of birth certificates, death certificates and marriage certificates can be requested by mail by providing an application, and a photocopy of positive identification including, but not limited to, a driver’s license, passport, or other government issued picture identification. Should you not have direct and legitimate interest in the record, you must complete the Release of Vital Records Application.

Send your application, a copy of your identification, along with a check payable to the City of Waterville, and a self-addressed, stamped envelope to:

City Clerk’s Office
One Common St.
Waterville, ME 04901

Fees for Certified Copies:

Certified copies are $15 for the first copy, and $6 for each additional copy (when purchased at the same time).

Vital requests are generally processed the same day they are received.
