Disclosure Statements
City Councilor and the Mayor
Every member of the City Council and the Mayor shall file a completed disclosure form, annually by no later than January 31st, concerning the official, the official’s spouse or the official’s registered or unregistered domestic partner, with the City Clerk, which must be posted on the city’s website by February 1st. Newly elected City Officials shall submit a disclosure form within 10 days of taking the oath of office. Officials are responsible for filing an updated disclosure form with the City Clerk, as necessary.
Appointed Members of the Planning Board, Zoning Board of Appeals & Board of Assessment Review
Appointed officials of the Planning Board, Board of Zoning Appeals and the Board of Assessment Review shall file a completed disclosure form, annually by no later than January 31st, concerning the official, the official’s spouse or the official’s registered or unregistered domestic partner, with the City Clerk, which must be posted on the city’s website by February 1st. Newly appointed members shall submit a disclosure form within 10 days of taking the oath of office. Appointees are responsible for filing an updated disclosure form with the City Clerk, as necessary.
Department Heads or Designees
The City Manager and all department heads of their designee shall file a completed disclosure form, annually by no later than January 31st, concerning the disclosing party, the disclosing party’s spouse or the disclosing party’s registered or unregistered domestic partner, with the City Clerk, with copies sent to the City Manager. Disclosure forms must be posted on the city’s website by February 1st. Newly hired department heads shall submit a disclosure form within 10 days of taking the oath of office. Department heads or their designees are responsible for filing an updated disclosure form with the City Clerk, as necessary.
